Did they ever determine what caused the TWO wheels of the front truck to derail?
The rail on the left side of the locomotive tilted over? Or did it knife the switch? Guard rail for the switch in the photo?

No answer that I ever saw, but that is normal reporting. I only see causes after major accidents, sometimes months or years after.
